AB The surveillance AIDS case definition in children, which was adopted by
the Ministry of Health (MOH)-Brazil in 1987, had low sensitivity and
utilized unrealistic criteria for diagnosis of opportunistic diseases.
WHO pediatric AIDS case definition has been found with low sensitivity
(21%), when tested in Brazil. In response to the need for a surveillance
definition more sensitive that not require special and expensive
laboratory tests, a simplified case definition was drafted through a
consensus of experts coordinated by the MOH. For epidemiological
surveillance a child (< 13 years) is considered to have AIDS if he has a
positive laboratory result for HIV, and at least two of the following
major signs or one major sign associated with at least two of the
following minor signs: Major signs: Oral candidiasis resistent to
regular therapy, Parotid chronic hyperplasia, Chronic or recurrent
diarrhea, Herpes zoster, Tuberculosis. Minor signs: Chronic or recurrent
otitis or sinusitis, Hepatomegaly or splenomegaly, Myocardiopathy,
Chronic dermatitis, Lymphadenopathy (> = 0.5 cm and > 2 sites),
Prolonged fever (> = 38 C and > = 1 month or recurrent), Weight loss (>
10% of baseline body weight) or abnormally growth curve, Anemia and or
lymphopenia and or thrombocytopenia. This definition presents the
following advantages: (1) simple to use; (2) includes tuberculosis and
(3) high specificity (by the incorporation of HIV serological testing).
It should be fully evaluated.
